New Patients

Please download, print, and fill out the required NEW PATIENT FORMS (see links below) and bring them to your visit. Please answer all applicable questions to the best of your knowledge and arrive at least 10 min prior to your office visit in order to complete the registration process without running into your scheduled appointment time. This information will help, while awaiting your medical records.

• If you are a new patient, please arrive at least 10-15 minutes early in order to complete the registration process.
• Please bring your insurance card to each visit. It is helpful if you know your office visit co-pay.
• Please bring all your medical records and vaccine records that you may have as well as any medications, herbals, vitamins and supplements your are taking.
• Please bring payment in the form of cash, check, or credit card (Visa, Master card, Discover, and American Express)

Ayurveda Patients

Please download, print, and fill out the required AYURVEDA PATIENT FORMS (see links below) and bring them to your visit. Please answer all applicable questions to the best of your knowledge and arrive at least 10 min prior to your office visit in order to complete the registration process without running into your scheduled appointment time.

• Please arrive at least 10 minutes early so not to feel visit is rushed.
• The Initial visit will include a thorough and detailed discussion of your medical history, eating and lifestyle habits. I will do an ayurvedic pulse measurement. Then, I will review your Ayurveda intake form with you and determine your mind-body type (Dosha). We will then discuss your body type meal plan and herbals.
• Follow up visits are generally 45 minutes, during which time we will discuss your progress.

Parking

Our office is located at 830 S Myrtle Ave,Monrovia, CA 91016. There is plenty of free parking in the front of the building and on the street.
